How to use this worksheet

  1. Check your lease for how the landlord wants repair requests sent (portal, email or address). Use that method.
  2. Fill in the date, your name, address and unit, and the landlord's or property manager's name and address.
  3. Describe each problem: what it is, where it is, and when it started. Use one line per problem.
  4. List earlier reports (date and how you reported them), if any.
  5. Write the repair you are asking for and a reasonable date for it to be done or scheduled, plus times you can provide access.
  6. List the photos or videos you are attaching, with the dates they were taken.
  7. Sign, date, and keep a copy along with proof of how and when you sent it.
